Late cancellations and no show policy
There is a strict 50% fee (of appointment cost) for the first late cancellation/no show appointment and a 100% fee for any subsequent occurrences. At least 24 hours notice is required when cancelling a scheduled appointment. Please understand that time is needed to offer the appointment to another patient. Emergency late cancellations and no shows will be taken into consideration and exceptions will be made accordingly.
Showing up late to an appointment
Showing up late to your appointment will result in partial loss of appointment time and sacrificed results. No refunds or discounts will be provided for loss of appointment time when the patient shows up late.
Medicare Disclaimer
Unfortunately, chiropractors are not able to “opt-out” of Medicare. This means that it is illegal for Medicare beneficiaries to self-pay for chiropractic services. Given that Health Performance is a self-pay clinic, this prevents us from treating Medicare patients altogether. We apologize for this disservice and hope that future reform occurs.
Thank you in advance for your cooperation!
